Employment Physicals
We’re able to perform physical examinations at any of our locations or at your workplace. We’ll examine and provide medical recommendations as well as supply appropriate paperwork necessary to start or return to work.

When non-life-threatening injuries occur on the job, our providers will get you the care needed to get back to work, along with paperwork to stay informed, as quickly and as safely as possible.

What to Expect
The Hometown Urgent Care Occupational Medicine program will simplify your responsibilities by making it easier for your company to remain compliant with regulatory agencies. Meanwhile, we can help you boost productivity by promoting a healthy, safe workplace environment.
Before Your Visit
To ensure a smooth visit, please bring the following:
- A photo ID
- Authorization form(s) from your employer
- Any company-specific paperwork that needs to be completed by our provider
CDL holders must also bring their driver’s license.
During Your Visit
If you meet with a provider, they’ll conduct a comprehensive exam to ensure you receive appropriate job placement or treatment. If you meet with a clinician, they’ll perform all necessary testing, keeping you informed every step of the way.
After Your Visit
All essential treatment paperwork and work status notes will be provided to you and your employer through an encrypted, secure method. Any necessary re-check appointments will be scheduled for you before you leave the clinic.

Please note that the following Occupational Medicine procedures will
only be offered daily until 7pm:
- Pre-employment and annual
employment physicals - Pre-employment drug screens
(regulated and non-regulated) - All other occupational medicine
services
Exceptions to the 7pm daily cutoff include:
- Post-accident, reasonable suspicion
and random drug and alcohol screens - Workers’ compensation injury care
